29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Jobsite, 1101 Somerset Avenue, Windber, PA 15963:  Observed on or about February 20, 2018, each employee working on a steep roof with a slope of approximately 9 in 12 (vertical to horizontal) was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  An employee was exposed to a fall hazard of approximately 20 feet to the ground below while installing a metal roof.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:   a) Jobsite, 1101 Somerset Avenue, Windber, PA 15963:  Observed on or about February 20, 2018, the employer did not maintain a certification record of fall protection training for each employee required to work at heights as part of their job.
