29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     On or about February 23, 2018, on the north side of the building where employees were exposed to a fall hazard greater than 6 feet when using a Werner gorilla ladder that was not secured at the top or have a grasping device.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about February 23, 2018 at the jobsite where employees were exposed to a fall hazard greater than 6 feet when accessing the roof to reach the equipment area to install metal panels without a fall protection system.       Swadley Roof Systems, LLC was previously cited for a violation of an equivalent occupational safety and health standard 1926.501(b)(11) which was contained in OSHA inspection number 997656, citation number 1, item number 1, and was affirmed as a final order on August 18, 2014 with respect to a workplace located at 3431 Nelson St., Lake Charles, LA 70605.
