OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards when damaged storage racks had not been removed from service:  On or about February 22, 2018, employees were exposed to struck-by hazards when rack supports/braces had been damaged and the racks had not been unloaded, repaired, or replaced.  Employees drive powered industrial trucks, walk, and work in the areas: a. Zone D107-damaged braces b. Zone D108-damaged brace c. Zone E130-damaged support leg d. Zone D118-damaged support leg e. Zone E137-damaged support leg f. Zone F149-damaged support leg, anchor bolt sheared off g. Zone E133-damaged support leg partially sheared off foot h. Zone E127-damaged brace  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.176(b): Storage of material created a hazard.  Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  On or about February 15, 2018, employees were exposed to struck-by hazards when the pallets on back-to-back racks can be pushed/moved by pallets on the opposite rack because the pallets touch each other in the flue space between the racks.  Product that is not fully banded and/or wrapped is subject to falling when jostled when a pallet on the opposite rack is being picked/stored.
