29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner.      A) KCS Contracting, LLC on or about February 22, 2018 at the address of 1458 Spring Meadows Drive, Holland, OH: The employer did not ensure employees locked the wheels of a mobile scaffold when working from the scaffold in a stationary location. Employees working from the scaffold in this condition were exposed to a fall and overturning hazards.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.      A) KCS Contracting, LLC on or about February 22, 2018 at the address of 1458 Spring Meadows Drive, Holland, OH: The employer did not ensure one employee working from a bakers scaffold was trained in the recognition of hazards associated with this type of scaffold. Fall hazards and unlocked wheels, went unrecognized while employee performed demolition operations from the scaffold.
