A) KCS Contracting, LLC on or about February 22, 2018 at the address of 1458 Spring Meadows Drive, Holland, OH: The employer did not ensure employees locked the wheels of a mobile scaffold when working from the scaffold in a stationary location. Employees working from the scaffold in this condition were exposed to a fall and overturning hazards.

A) KCS Contracting, LLC on or about February 22, 2018 at the address of 1458 Spring Meadows Drive, Holland, OH: The employer did not ensure one employee working from a bakers scaffold was trained in the recognition of hazards associated with this type of scaffold. Fall hazards and unlocked wheels, went unrecognized while employee performed demolition operations from the scaffold.
