29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a): Teddy Graham Department- Conveyor CVE 20- An employee was exposed to mechanical energy from the conveyor system that was being cleaned. The employer failed to ensure that employees followed established procedures regarding the control of hazardous energy. The employer failed to ensure that the employee was following the procedure to lock out the energy sources for the moving components of the conveyer system before beginning cleaning. The employee sustained a right arm fracture due to contact with moving parts of the conveyor system, on or about March 1, 2018.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
