29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about March 9, 2018, at 9400 W Commercial Blvd., Sunrise, FL 33351, employees were exposed to a fall hazards while installing/removing roofing material on an overhang, while wearing a personal fall arrest system, wherein the system was rigged such that during a fall, the employees would free fall more than 6 feet and/or the system was rigged such that the employee would have contacted the lower level.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).  On or about March 9, 2018, at 9400 W Commercial Blvd., Sunrise, FL 33351, the employer did not provide fall protection training to the employees exposed to a 10-feet 8 inch fall hazard while installing/removing roofing material on an overhang.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about March 9, 2018, at 9400 W Commercial Blvd., Sunrise, FL 33351, employees were exposed to a 10-feet 8-inch fall hazard while installing/removing roofing material on an overhang, while not being protected from falling to the lower level.   The Alpha Roofing and Sheet Metal LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 129 CFR 926.501 (b)(1), which was contained in OSHA inspection number1260489, citation number 2, item number1 and was affirmed as a final order on 02/15/2018, with respect to a workplace located at 190 SW Port Saint Lucie Boulevard, Port Saint Lucie, FL 34984.
