29 CFR 1910.146(d)(4): Provide the following equipment (specified in paragraphs (d)(4)(i) through (d)(4)(ix) of this section) at no cost to employees, maintain that equipment properly, and ensure that employees use that equipment properly:   a)  On or about February 14, 2018 employees were required to enter into permit required spaces on the site of the facility without the proper testing equipment.   The test equipment on hand hadn't been kept up and was not functional. 15 of the 16 permit required spaces were identified as having a potentially hazardous atmosphere and required the  atmosphere inside the space to be monitored and tested, as stated in the written confined space program, dated April 2008.

29 CFR 1910.146(g)(3): The training shall establish employee proficiency in the duties required by this section and shall introduce new or revised procedures, as necessary, for compliance with this section.  a) On or about February 14, 2018 employees were required to enter permit required spaces.  The spaces identified in the written program, dated April 2008, identified 16 permit required spaces with hazards that included but were not limited to atmospheric and those requiring fall arrest equipment. Initial training provided to employees when program was established and no further training provided to maintain proficiency in practices such as but not limited to emergency retrieval procedures.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. [63 FR 33450, June 18, 1998]  a)  On or about February 14, 2018 employees handled, applied and mixed chemicals and or pesticides such as but not limited to Roundup, containing Glyphosate, on the facility's grounds to control and kill vegetation.  Serious eye damage may occur based on the Global Harmonizing Standard (GHS) "Danger" and as defined in the National Institute of Health's (NIH) PubChem chemistry database.  Possible loss of eyesight may result without immediate flushing of the eyes and follow up medical treatment.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met.  a)  On or about February 14, 2018, employees did not have a written hazard communication program to refer to, but not limited to, for guidance on the program practices , and for informing and training on the safe measures for using potentially hazardous substances. Departments such as the Environmental Services had safety data sheets on hand listed on an inventory without a written Hazard Communication Program.

29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:  a)  On or about February 14, 2018 employees working in areas such as but not limited to Environmental Services and those performing landscaping duties hadn't been received training and information of the potential risks that were present in their job with using a documented assessment.  Environmental Services employees used gloves when cleaning with chemicals and landscaping staff  wore face shields and hearing protection when operating outdoor power tools.

29 CFR 1910.1030(c)(1)(i): Each employer having an employee(s) with occupational exposure as defined by paragraph (b) of this section shall establish a written Exposure Control Plan designed to eliminate or minimize employee exposure.  a)  On or about February 14, 2018 A written Exposure Control Plan was not available for employees to reference for guidance on how to practice safe work practices to prevent contracting HIV and or hepatitis B.  The written procedures that departments such as but not limited to the Housekeeping department for the lodge had didn't include all of the required information such as a section for exposure determination.
