On or about March 28, 2018, the employer did not provide additional medical evaluations for employees prior to employees wearing a respirators. An employee stated that it has been more than three years since he has been provided a medical evaluation.

On or about March 28, 2018, a metallic outlet box (handy box) was not installed in accordance with the UL identifier QCIT which states that boxes shall be mounted in or on a surface.  The handy box was attached to a drop down flexible cord and suspended in the air.

29 CFR 1910.305(g)(2)(ii): Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.    a.  On or about March 28, 2018, the employer used a flexible cord that had a splice which did not retain the insulation or outer sheath properties approximately one inch from the end of the metal pipe going through the wall into the Shell Department.  The flexible cord was used to provide power to the temporary lights in the North Permanent Mold room during the remodeling project taking place.

29 CFR 1910.1200(f)(6)(ii): The employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.    a.  On or about March 28, 2018, the employer did not ensure that secondary containers of hazardous chemicals were labeled, tagged, or marked with product identifier and words, pictures, symbols, or combination thereof which will provide employees with general information regarding the hazards of the chemicals.  Containers of adhesives and release agents used on the molds and cores in the Free Flow Area were unlabeled and being used by employees.

You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.  a.  On or about March 28, 2018, the employer who had over 250 employees during 2017 and was in an industry listed in appendix A to subpart E of this part, did not electronically submit their OSHA Form 300A to OSHA prior to the due date of December 15, 2017.
