29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level: On or about May 30, 2018, the employer failed to keep free and unobstructed, the emergency exit route that led to the emergency exit door located in Plant #4 in the far right side of the building; past the Gerber Machine. -

The emergency exit door located in Plant #4 in the far right side of the building past the Gerber Machine did not have a sign above the door. -

29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: On or about March 29, 2018, the employer failed to ensure that the procedures developed for locking and tagging out Line #5 Tie Extruding Machine located in plant #3, included testing the machine once it was locked out to ensure all of the energy sources were isolated prior to working on the machine. Employees are exposed to electrical energy on this machine when performing tasks such as but not limited to heater plate change outs. NOTE: The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1177577, citation 1, item 1c, which was affirmed as final order on January 1, 2017, with respect to the workplace located at 4602 Benefit Ave., Ashtabula OH 44004.
