Rig #1. On or about April 02, 2018, employees were operating an oil/gas well service rig in support of the serving of an oil/gas well. One employee worked on the rig derrick rod basket where the secondary means of emergency egress, the Geronimo system, had not been installed. This exposed the employee to the hazards of fall from heights.      Mills Well Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSH Act of 1970 Section 5(a)(1), which was contained in OSHA inspection number 891114, citation number 01, item number 01 and was affirmed as a final order on June 10, 2013, with respect to a workplace located at Yount B #1 Well, Sec 2-11N-3W Oklahoma City, OK 73128.
