29 CFR 1910.147(a)(2)(ii)(B): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation. On or about March 26, 2018 and times prior there to, the employer did not develop and implement a written, machine specific procedure to control hazardous energy for the #867 Bender Machine when operators were performing minor servicing such as, but not limited to changing out the dies that require the operator to place their hand in the point of operation of the hydraulic ram.
