29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Employees perform maintenance and servicing activities including but not limited to repairs and clearing jams on equipment such as, but not limited to machine 4065. Employees were exposed to being caught in the point of operation. (a) Operators clearing jams and/or removing unprocessed parts did not utilize lockout or tagout devices to isolate energy sources. The interlocked doors did not provide adequate protection in that they were not fail-safe. (b) Maintenance employees performing repairs such as, but not limited to correcting sensor malfunctions did not utilize lockout or tagout devices and were not afforded adequate alternative protection when interlock keys were utilized to bypass the machine guarding.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: Employees performed servicing and maintenance activities on machine 4065. The employer did not ensure a periodic inspection of the energy control procedure was conducted to identify deficiencies such as, but not limited to missing statement of the intended use of the procedure, deficiencies in the verification procedures, and missing specific procedural steps for removal of the energy isolation devices. Employees were exposed to hazards associated with the unexpected release of electrical, hydraulic, and pneumatic energy.
