Employees performing servicing and maintenance did not receive training adequate for authorized employees, exposing the employee to hazards such as, but not limited to being struck-by and caught-in the machine.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about November 15, 2023, and at times prior thereto, employees performed maintenance activities including, but not limited to clearing jams on equipment such as, but not limited to the press bender #867. Energy control procedures did not include specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures, exposing employees to hazards such as, but not limited to being struck-by and caught-in the machine. Elkhart Products Corporation was previously cited for violations of this occupational safety and health standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1506149, Citation 1, Item 1 affirmed as a final order on July 7, 2021, with respect to a workplace located at 3265 S School Ave, Fayetteville, Arkansas.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about November 15, 2023, throughout the facility, employees were performing service and maintenance on machines and equipment without the employer performing annual inspections of the energy control procedures to identify deficiencies, exposing employees to hazards such as, but not limited to being caught-in and struck-by components of machines.
