On or about February 15, 2018, employees of Omar Construction USA, Inc. were using to access the roof a Werner 10-foot ladder that was not able to be extended 3 feet above the landing.  The ladder was not secured to prevent accidental displacement and a grasping device was not provided to assist employees in mounting and dismounting the ladder, exposing employees to a fall hazards.
