On January 31, 2019, in the trusses above the 3rd level, employees were not adequately protected from a 27 feet fall hazard by the use of a fall protection system in that the slack of the rope would allow them to fall more than 6 feet and up to 27 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On January 31, 2019, the portable step ladder used to access the 2nd level did not extend 3 feet above the landing, exposing employees to a 9 feet fall hazard. Paradise Builders of North Florida was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1306360, citation number 1, item number 1 and was affirmed as a final order on May 23, 2018, with respect to a workplace located at 9874 Soldier Court, Jacksonville, Florida 32221.
