29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  (a) New residential structure located on Lot #203, Front side: On or about 04/05/2018, and at times prior, the employer exposed employees to falls of up to 13-feet to the lower level, in that the ladder used to access the roof was not set at the proper angle. A safe angle of 75-1/2 degrees is recommended by the manufacturer and the ladder on site was placed at a 50-degree angle.

Fenix General Solution, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1207609, citation number 1, item number 1 and was affirmed as a final order on 03/13/2017, with respect to a workplace located at 325 Harvest View Terrace, Woodstock, GA 30188.
