29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    Note:  The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.      a).    On or about 4/11/18, on site, for employees handling Rhino Hybrid 21-60 Resin Clear and Rhino Hybrid 21-60          Isocynate during bed liner spraying. The Rhino product contains          chemicals such as, but not limited to, Diphenylmethane Diisocyanate (MDI) and Polyether Polyol. The employer did not provide a medical evaluation for the employees required to don a supplied air respirator.     b).    On or about 4/11/18, on site, for employees handling Z Guard Clear Sealant when conducting undercoat spraying           tasks. The employer did not provide a medical evaluation for the employees required to don an R95 on a daily basis.                       .

On or about 4/11/18, on site, for employees handling Z Guard Clear Sealant when conducting undercoat spraying          tasks. Employees are required to don an R95 on a daily basis. The employer did not provide a respiratory protection          program to include a fit testing.

The Rhino product contains         chemicals such as, but not limited to, Diphenylmethane Diisocyanate (MDI) and Polyether Polyol. Employees are         required to don a supplied air respirator. The employer did not provide a respiratory protection training.   b).    On or about 4/11/18, on site, for employees handling Z Guard Clear Sealant when conducting undercoat spraying          tasks. 29 CFR 1910.134(i)(1)(ii): The Employer did not ensure that compressed breathing air met at least the requirements for Grade D breathing air described in ANSI/Compressed Gas Association Commodity Specification for Air, G-7.1-1989, to include:   (A): Oxygen content (v/v) of 19.5-23.5%;   (B): Hydrocarbon (condensed) content of 5 milligrams per cubic meter of air or less;   (C): Carbon monoxide (CO) content of 10 ppm or less;   (D): Carbon dioxide content of 1,000 ppm or less; and   (E): Lack of noticeable odor.    a) On or about 4/11/18, on site, for employees required to don a supplied air hood when spraying bed liners. The employer did not test the air to make sure it comply with Grade D air.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:      a).    On or about 4/11/18, on site, for employees handling Rhino Hybrid 21-60 Resin Clear and Rhino Hybrid 21-60          Isocyanate during bed liner spraying. The Rhino product contains chemicals such as, but not limited to,           Diphenylmethane Diisocyanate (MDI) and Polyether Polyol. These chemicals are corrosive to the skin and are          sensitizing to the skin. The employer did not provide a hazard communication training.     b).    On or about 4/11/18, on site, for employees handling Z Guard Clear Sealant when conducting undercoat spraying           tasks. Z-Gard Clear Sealant contains chemicals such as, but not limited to, mineral spirits. This chemical causes skin          and eye irritation. The employer did not provide a hazard communication training.
