29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a) On or about 4/10/18 at a roof replacement project located at 49 Jamaica Lane, Cheektowaga, New York; employees installing roofing were using ropes with rope grabs attached to their harnesses. The employees did not have their rope grabs adjusted or rigged to prevent free falls more than 6 feet.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 4/10/18 at a roof replacement project located at 49 Jamaica Lane, Cheektowaga, New York; employees were placing ice shield on the lower roof of a two story house. The roof had a 5 in 12 pitch and an eave height of approximately 10 feet. There was no fall protection in use.  NO ABATEMENT CERTIFICATION REQUIRED  Krieb Roofing & Siding LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1258901, citation number 1, item number 1 and was affirmed as a final order on 9/25/17, with respect to a workplace located at 125 Arielle Court, Williamsville, New York.
