29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  At the east entrance, the handrail on the left side of the stairway (steps) that ascended up to the door had broken loose from the stairs near the stairs bottom, creating a slip, trip, fall, hazard.

29 CFR 1910.22(d)(1): (1) Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition:  At the east entrance, the handrail on the left side of the stairway (steps) that ascended up to the door was not inspected as necessary to determine it had broken loose from the stairs near the stairs bottom, and was not maintained in a safe condition.

29 CFR 1910.22(d)(2): (2) Hazardous conditions on walking-working surfaces were not corrected or repaired before employees used the walking-working surface again:    At the east entrance, the handrail on the left side of the stairway (steps) that ascended up to the door had broken loose from the stairs near the stairs bottom.  The handrail was not repaired before employees used the stairway again.

29 CFR 1910.36(d)(3):  Exit route doors were locked from the inside, not only in mental, penal, or correctional facilities:  a)  The southeast exit door was locked from the inside.    b)  The northeast exit door leading into the east entry/exit route was locked from the inside.  The US Department of Agriculture-FSA was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.36(d)(3), which was contained in OSHA inspection number 1167409, Notice number 1, Item number 3, issued on 09/06/2016, and became a Final Order on 09/28/2016.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful.  The abatement certification sheet is enclosed with the notice(s).
