29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). 1) South end of trench: On or about April 19, 2018, the employer exposed employees to a cave-in hazard, in that employees were working inside a trench with nearly vertical walls approximately 9 feet deep, 25 feet wide and 69 feet long grouting an 18 inch pipe without adequate cave-in protection.
