29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: A) Continental Structural Plastics, Inc., located at 2915 County Highway 96, Carey, OH 43316: On or about April 2, 2018, the employer did not ensure that periodic inspections of the energy control procedures for the hydraulic press # 3 (Lawtomatic 1200 Ton Press; Manufacturers ID # 12001370) were conducted at least annually. Employees did not follow all the steps required for the control of the hazardous energy sources prior to changing dies in the presses. Employees were exposed to struck-by and amputation hazards while all hazardous energy sources were not controlled. B) Continental Structural Plastics, Inc., located at 2915 County Highway 96, Carey, OH 43316: On or about April 24, 2018, the employer did not ensure that periodic inspections of the energy control procedures for the hydraulic press # 9 (Williams & White 2000 Ton Press; Manufacturers ID # C-4269) were conducted at least annually. Employees did not follow all the steps required for the control of the hazardous energy sources prior to changing dies in the presses. Employees were exposed to struck-by and amputation hazards while all hazardous energy sources were not controlled. Employees conducted set-up activities such as, but not limited to, changing dies in the hydraulic press # 3 (Lawtomatic 1200 Ton Press; Manufacturers ID # 12001370). Employees were exposed to struck-by and amputation hazards while all hazardous energy sources were not controlled prior to conducting work. Employees were exposed to struck-by and amputation hazards while all hazardous energy sources were not controlled prior to conducting work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from struck-by and amputation hazards while employees conducted activities such as, but not limited to, changing dies in hydraulic presses # 3, # 9, and # 12.

Multiple legs of the shelving unit, located in the die storage area, were damaged. Employees, working in the area, were exposed to struck-by hazards from equipment stored on the shelves and the shelving itself.

Employees conducted set-up activities such as, but not limited to, changing dies and were exposed to struck-by and amputation hazards while all hazardous energy sources were not controlled prior to conducting work.
