29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    A) Continental Structural Plastics, Inc., located at 1276 Industrial Drive, Van Wert, OH 45891: On or about June 13, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed prior to clean-up activities. Employees conducted activities such as, but not limited to, cleaning the Ronningen-Petter DCF filter system (Model DCF-800, Serial # 393321.001-1) located at the SMC (sheet molding compound) machine # 2. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.      As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from amputation hazards while employees conducted activities such as, but not limited to, cleaning the Ronningen-Petter DCF filter system (Model DCF-800, Serial # 393321.001-1) located at the SMC (sheet molding compound) machine # 2.
