29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a. On or about April 26, 2018, located at 1020 Coshocton Avenue in Mount Vernon, Ohio, employees were installing metal roofing material onto a Star Brite Car Wash at heights between approximately 12 and 14 feet without fall protection. The employer never initiated or maintained safety and health programs to protect employees from the hazards associated with performing roofing work on a steep-slope roof.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a. On or about April 26, 2018, located at 1020 Coshocton Avenue in Mount Vernon, Ohio, employees were installing metal roofing material onto a Star Brite Car Wash at heights between approximately 12 and 14 feet without fall protection. The employer never instructed employees in the recognition and avoidance of unsafe conditions with the regulations applicable to protect employees from the hazards associated with ladder use and performing roofing work on a steep-slope roof.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a. On or about April 26, 2018, located at 1020 Coshocton Avenue in Mount Vernon, Ohio, employees were performing roofing work on a 6 in 12 pitch roof without any means of fall protection, thereby exposing the employees to a fall hazard between approximately 12 and 14 feet.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).   a. On or about April 26, 2018, located at 1020 Coshocton Avenue in Mount Vernon, Ohio, employees were using a Werner 24 foot aluminum extension ladder at too shallow of an angle. The working length of the ladder was approximately 18.5 feet.  The horizontal distance from the top support to the foot of the ladder was approximately 11 feet when it was required to have been between 4 and 5 feet.

On or about April 26, 2018, located at 1020 Coshocton Avenue in Mount Vernon, Ohio, three employees were working off of a Werner 24 foot aluminum extension ladder at the same time.  The employees' combined weight exceeded the ladder's maximum capacity of 350 pounds.

On or about April 26, 2018, located at 1020 Coshocton Avenue in Mount Vernon, Ohio, employees were carrying and handling sheets of metal roofing material that were approximately ten feet long, while climbing on top of a Werner 24 foot aluminum extension ladder. The employees were not maintaining at least three points of contact with the ladder which exposed them to a fall hazard.
