29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  a) On or about May 10, 2022, employees were observed during the roofing operations of a residential structure located at 6607 Dora Blvd, INDEPENDENCE, OH 44131; the employer did not ensure the employees working on the ground level were protected from the hazard of head injury from impact, or from falling or flying objects by means of a protective helmet.

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a) On or about May 10, 2022, employees were observed using a ladder during the roofing operations of a residential structure located at 6607 Dora Blvd, INDEPENDENCE, OH 44131; the employer did not ensure the ladder side rails were extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access to the roof.

29 CFR  1926.1053(b)(13):The top or top step of a stepladder shall not be used as a step.  a) On or about May 10, 2022, during work on a residential roof located at 6607 Dora Blvd, Independence, OH 44131, the employer did not ensure that employees were protected from a fall of 9.1 feet by ensuring they did not use the top or top step of a stepladder for access to the roof.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)	On or about May 10, 2022, during a roofing project located at 6607 Dora Blvd, INDEPENDENCE, OH 44131; the employer did not ensure that employees on the roof were protected from a fall of 9.1 feet by utilizing guardrail systems, safety net system, personal fall arrest system, or alternative fall protection measures.  Walnut Creek Companies, LLC DBA Walnut Creek Metals was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1311541, citation number 1, item number 3 and was affirmed as a final order on 6-12-2018, with respect to a workplace located at 1020 Coshocton Ave, Mount Vernon, Ohio 43050.
