29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  On or about September 16, 2025, at a job site located at 3400 Lincoln Way East, Massillon Ohio 44646; during the installation of roofing material on a residential structure, the employer did not ensure all employees using an aerial lift were protected from fall hazards approximately 19.5 feet by a fall protection device.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about September 16, 2025, during a roofing project located at 3400 Lincoln Way East, Massillon Ohio 44646; the employer did not ensure that employees on the roof were protected from a fall of 19.5 feet by utilizing guardrail systems, safety net system, personal fall arrest system, or alternative fall protection measures.  Walnut Creek Companies, LLC DBA Walnut Creek Metals was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1632823, citation number 2, item number 1 and was affirmed as a final order on 12/6/2022, with respect to a workplace located at 6607 Dora Blvd, INDEPENDENCE, OH 44131.

29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:  On or about September 16, 2025, employees were observed using a ladder during the roofing operations on a residential structure located at 3400 Lincoln Way East, Massillon Ohio 44646; the employer did not ensure the ladder side rails were extended at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access to the roof.  Walnut Creek Companies, LLC DBA Walnut Creek Metals was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1632823, citation number 1, item number 2a and was affirmed as a final order on 12/6/2022, with respect to a workplace located at 6607 Dora Blvd, INDEPENDENCE, OH 44131.
