On or about April 30, 2018, employees were exposed to a struck by hazard. The employees were performing roofing activities on a residential home located on S/L 40 Mavona, North Ridgeville, Ohio 44139. Employees were working on lower levels underneath other employees without wearing a protective helmet.

On or about April 30, 2018, employees were exposed to a struck by hazard. The employees were performing roofing activities on a residential home located on S/L 40 Mavona, North Ridgeville, Ohio 44139. Employees were using air powered nail guns without wearing protective safety glasses.

The employees were working on all areas of the roof without using any form of fall protection.
