A.)  DH Steel Products, LLC, 121 West 10th Street, Erie, PA, on or about May 4, 2018 and times prior thereto:  A JLG 600S aerial lift (S/N:  035967 0300026435) was being used for work operations.  A flexible cord on the aerial lift that was available for use to supply power to electrical receptacles within the basket of the aerial lift was frayed at the cord cap.  The flexible cord and/or aerial lift was not tagged out of service, locked out, rendered in operable or physically removed from service.
