29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: (a) On the south east corner of the roof of the Tower Building equipment worked on was located approximately 5 feet 10 inches from the edge of roof. No fall protection was provided in this area. (b) On the south west side of the Tower Building roof there is an approximate 20 foot space along the roof edge extending to the HVAC unit. No fall protection was provided until immediately next to the unit.
