29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. a) Facility, 79 Park Lane, Hegins, PA - Employees operated a UniCarriers Forklift Model FCG30HL-A1 (Serial No. CU1F2-9Q2540) with a front-end attached Cascade D-Clamp Model 70D-FPS-142 (Serial No. 266611-1) without the manufacturer's prior written approval exposing employees to struck-by hazards created by the modifications to the powered industrial truck's lifting capacity and center of gravity, as observed on or about May 10, 2018. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
