29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Department 4300 - Employees hand-fed window sashes into a Sampson sash cleaner. The fixed barrier guard on this machine had a height of 3 inches above the table, which would allow employees' fingers and hands to reach the nearest hold-down clamp located a distance of 7.125 inches back from the barrier guard, exposing employees to a struck-by hazard from the hold-down clamp, on or about January 8, 2020. (b) Department 3500 - Employees hand-fed window sashes into a Rotox sash cleaner. The fixed barrier guard on this machine had a height of 6.25 inches above the table, which would allow employees' fingers and hands to reach the nearest hold-down clamp located a distance of 20 inches back from the barrier guard, exposing employees to a struck-by hazard from the hold-down clamp, on or about January 8, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
