On or about May 9, 2018, the employee handing sheathing up to another employee on the roof was not wearing a hart hat and being exposed head injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a. On or about May 9, 2018, roof area, the employee sheathing a 6:12 pitch roof was not protected from falling up to a 29 feet fall hazard by the use of a fall protection system. b. On or about May 9, 2018, stairs leading to second floor, did not have a guardrail system, exposing employees to a 10 feet fall hazard.
