29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a. On or about September 26, 2019, at the jobsite, on the west side of the front entrance, employees applying plaster, to the exterior wall of a commercial building, were exposed to ejection hazards while working from a JLG 660SJ aerial lift. The employees were working without the use of a fall arrest system such as a full body harness with a safety lanyard or a body belt used as a restraint system. b. On or about September 26, 2019, at the jobsite, on the east side of the front entrance, employees applying plaster, to the exterior wall of a commercial building, were exposed to ejection hazards while working from a JLG 660SJ aerial lift. The employees were working without the use of a fall arrest system such as a full body harness with a safety lanyard or a body belt used as a restraint system.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold (including aerial lifts) trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a. On or about September 26, 2019, at the jobsite, untrained employees were working from and operating two JLG660 SJ aerial lifts. The employees had not been trained on the safe operating procedures for the aerial lifts and they were exposed to ejection hazards.

On or about September 26, 2019, at the jobsite, untrained employees working from aerial lifts were exposed to ejection hazards. The employees were wearing safety harnesses over their shoulders, without using the legs straps, and the employees were not using safety lanyards.
