29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    At this establishment, the employer did not have an effective respiratory protection program in that the program lacked the following:    a.  designating a qualified respiratory protection program administrator;  b.  procedures for selecting respirators for use in specific locations or doing specific tasks, duties, or activities;  c.  designating a medical provider who will evaluate medical evaluation questionnaires and conduct the medical exam;  d.  designating a non-managerial employer representative to assist non-readers or non-English-reading employees;  e.  designating the method utilized for fit-testing;  f.  identifying a method for respirator storage, cleaning, maintenance and repair;  g.  monitoring proper respirator use during routine activities;  h.  identifying the individual responsible for conducting respirator training before employees wear their respirators and annual training thereafter;  i.  developing procedures for regularly evaluating the effectiveness of the respiratory program; and,  j.  where the records will be kept.
