On or about May 17, 2018, employees installing shingles on the roof of a residential home located at 11981 Apollo Drive North Royalton, Ohio 44133 were using pneumatic nail guns and the employer did not ensure employees were provided with and used eye protection.

The employer did not ensure employees were protected through the use of fall protection systems/devices.
