29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about August 3,2020, employees were exposed to a fall hazard while performing roofing activities on a residential home located at 261 Orchard View, Seven Hills, Ohio 44131. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about August 3,2020, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 261 Orchard View, Seven Hills, Ohio 44131. The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses. Michael Rogers Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102 (a)(1) which was contained in OSHA inspection number 1316705, citation number 1, item number 1 and was affirmed as a final order on 5/17/2018, with respect to a workplace located at 11981 Apollo, North Royalton, Ohio 44133.

The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection. Michael Rogers Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1316705, citation number 1, item number 2 and was affirmed as a final order on 5/17/2018, with respect to a workplace located at 11981 Apollo, North Royalton, Ohio 44133.
