29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):     (a)New Angus, LLC dba DemKota Ranch Beef at 13 135th Street South West, Aberdeen, SD 57401: On or about and at times prior to May 14, 2018, the employer did not ensure that machine specific Lock-out/Tag-out procedures associated with multiple energy source machines were developed to protect employees from inadvertent machine start up. On May 14, 2018, a QA inspector received a left hand amputation injury involving the index, middle and ring finger after placing his hand on the side puller machine when the machine actuated during an inspection. The side puller machine was re-energized by a maintenance worker located in the basement of the facility prior to the completion of pre-op work inspections. When the side puller is hydraulically re-energized, the paddles of the machine automatically activated and cycled. The employer did not have specific procedures related to de-energizing the electrical functions related to the machine which would have disabled the paddles from cycling. This condition exposed employees to amputation injuries.          Abatement Note:  The procedures shall include but are not limited to the following:       (1)        A specific statement of the intended use of the procedures;       (2)        Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;       (3)        Specific procedural steps for the placement, removal and transfer of lockout devices or tag-out devices and the responsibility for them; and       (4)        Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tag-out devices, and other energy control measures.
