29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):      a)  On May 23, 2018, employees were not protected from the hazard of cave-in while working in a trench measuring approximately 7 feet in depth.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:      a) On June 19, 2018, employees working in a trench were not wearing protective helmets where there was a possible danger of head injury from falling objects.  Goose Lake Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1189653, citation number 1, item number 2 and was affirmed as a final order on December 14, 2016, with respect to a workplace located at 374 Lee St., Des Plaines, Illinois 60016.  Goose Lake Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1162229, citation number 1, item number 2 and was affirmed as a final order on August 16, 2016, with respect to a workplace located at 2515 W. Lawrence Ave., Chicago, Illinois 60606.  No abatement certification or documentation is required for this item.
