29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles and/or materials:    (a)Heavy Construction, Inc. at 800 Dynamic Avenue, Harrisburg, SD 57032: On and about and at times prior to May 30, 2018, the employer did not ensure employees were protected from eye hazards in that, employees were using pneumatic nail guns and were not provided eye protection. This condition exposed employees to eye injuries.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.:    (a)Heavy Construction, Inc. at 800 Dynamic Avenue, Harrisburg, SD 57032: On and about and at times prior to May 30, 2018 the employer did not ensure employees were protected from fall hazards in that, employees installing purlins were not utilizing a fall protection system. This condition exposed employees to fall hazards at an approximate height of 16-27 feet above a lower level.

This condition exposed employees to fall hazards of approximately 16 feet above a lower level.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more:    (a)Heavy Construction, Inc. at 800 Dynamic Avenue, Harrisburg, SD 57032: On and about and at times prior to May 30, 2018, the employer did not ensure employees were protected from fall hazards when descending from the roof in that, a ladder or other means of appropriate egress were not provided. Employees were climbing down wood framing to get to the ground level. This condition exposed employees to fall hazards of up to 16 feet to a lower level.
