29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: On or about 05/11/2017, at the construction site, located at 501 Olive Blvd., St. Louis, Missouri, the following floor holes were observed uncovered. a) On the 4th floor, in the former electrical chase area, near the east wall, a floor hole measuring 6 inches X 8 inches was not covered. b) On the 4th floor, in the former electrical chase area, on the west side, a floor hole measuring 5 inches X5 inches was not covered. c) On the 5th floor, in the former electrical chase area, near the east wall, a floor hole measuring 5 inches X 15 inches was not covered. d) On the 5th floor, in the former electrical chase area on the west side a floor hole measuring 2.5 inches X 18 inches was not covered.
