29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about, 05/31/2018 in Building 6 Chanel KEL, employees were exposed to flying debris hazards when operating the EGR39 #2 with an opening on the side of the machine.  b) On or about, 05/31/2018 in Building 1 Separator, employees were exposed to caught-by hazards when operating the 10 ton broach without a guard covering moving shaft.  c) On or about, 05/31/2018 in Building 1 Lathe Department, employees were exposed to rotating hazards when operating the lathes without a guard.  d) On or about, 05/31/2018 in Building 1 Maintenance Shop, employees were exposed to laceration hazards when operating the bandsaw without guarding the bottom portion of the blade. 29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  a) On or about 05/31/2018 throughout establishment, when maintenance employees perform servicing or maintenance such as but not limited to changing the pneumatic filter on the Channel 1 EGR05, a group lock out procedure was not followed to ensure every employee performing this maintenance applied their own lock to the group lockout device.
