29 CFR 1910.147(c)(7)(i):  The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:      (a)   Steven Roberts Originals, LLC at 2708 Tower Road, Aurora, CO 80011:  On and before May 31, 2018, the employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees in that the instructions concerning energy control were general.  This condition exposed the employees to the hazards of improper lockout procedures for, but not limited to, the equipment and machines in the production area.

29 CFR 1910.212(a)(1):  Machine guarding was not provided to protect operators and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a) Steven Roberts Originals, LLC at 2708 Tower Road, Aurora, CO 80011:  On and before May 31, 2018, the employer did not ensure that machine guarding was provided to protect operators and other employees from a machine hazard, in that the pneumatic piston driving the brownie batter dispenser exposed employees to struck-by or caught in between hazards. This condition exposed the employees to crushing and amputation hazards.      (b) Steven Roberts Originals, LLC at 2708 Tower Road, Aurora, CO 80011:  On and before May 31, 2018, the employer did not ensure that machine guarding was provided to protect operators and other employees from hazards created by ingoing nip points in that the high speed conveyor belt and pulley on the Core Cake Pop dispenser, next to RS1, exposed employees to mechanical hazards due to it being unguarded. This condition exposed the employees to amputation hazards.      (c)  Steven Roberts Originals, LLC at 2708 Tower Road, Aurora, CO 80011:  On and before May 31, 2018, the employer did not ensure that machine guarding was provided to protect operators and other employees from hazards created by rotating parts, in that the chocolate return/recirculation catch basin (M15) on the Cake Bar Line exposed employees to rotating parts hazards due to the unguarded basin with motor driven paddles inside. This condition exposed the employees to an amputation hazard.      (d) Steven Roberts Originals, LLC at 2708 Tower Road, Aurora, CO 80011:  On and before May 31, 2018, the employer did not ensure that machine guarding was provided to protect operators and other employees from hazards created by rotating parts, in that the chocolate return/recirculation catch basin on the Premium Cake Pop Line exposed employees to rotating parts hazards due to the unguarded basin with motor driven paddles inside. This condition exposed the employees to an amputation hazard.

This condition exposed the employees to the hazard of rotating parts.

29 CFR 1910.219(c)(3): Vertical or inclined shafting seven -7 feet or less from floor or working platform was not enclosed with stationary casing(s):          (a) Steven Roberts Originals, LLC at 2708 Tower Road, Aurora, CO 80011:  On and before May 31, 2018, the employer did not ensure that the vertical shafting underneath the chocolate melt basin (CME006) on the Cake Bar Line was protected by a stationary casing in accordance with requirements of paragraphs (m) and (o) of this section. This condition exposed the employees to the hazard of rotating parts.
