29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done: a) On or about June 6, 2018, at the above located workplace, employees were required to use personal protective equipment such Ansell, HyFlex, 11-72 R, 8X; and Rugged Wear, Rubber-Dipped, 660-1216 gloves, while working with rough (unfinished) Oak and Pine wooden pallets, industrial strength cleaners, such as Member's Mark Commercial Lemon Fresh Disinfectant Cleaner and other chemicals. The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done.
