29 CFR 1910.36(d)(2): Exit route doors were operated by a device or alarm that could restrict emergency use of the exit route if the device or alarm fails: a) Second Floor, Entrance to Police Offices - On or about August 9, 2018, in the event of a fire emergency, a magnetically controlled lock of a access-controlled egress door leading to an office suite entrance door would not unlock automatically upon the building's fire-protection signaling system being activated or in the event of a power failure. b) Second Floor, Entrance to Human Recourses Department - On or about August 9, 2018, in the event of a fire emergency, a magnetically controlled lock of a access-controlled egress door leading to an office suite entrance door would not unlock automatically upon the building's fire-protection signaling system being activated or in the event of a power failure. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.37(b)(4): When the direction of travel to the exit or exit discharge was not immediately apparent, signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge, and the line-of-sight to an exit sign was not clearly visible at all times: a) First Floor, Access Route to North Stairway - On or about August 9, 2018, exit signage indicating the location of an exit (a stairway) was not visible from the hallway leading to the exit. b) Second Floor - On or about August 9, 2018, exit signage indicating the location of an exit was not visible from the hallway leading to the exit. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
