29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: a) Solar Pad #26 and #34 Transformers - An electrician de-energized the secondary side of four transformers and did not provide a tag on the selector switch, 277/480 volt service, on or about 09/28/2018. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
