29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a) On or about June 11, 2018, and at times prior, White Roofing, Inc. as the controlling, creating, and exposing employer did not ensure that employees of G&G Roofing, LLC., a subcontractor, used a body belt or a body harness with a lanyard attached to the boom or basket while riding on an aerial lift. Employees were exposed to falls of up to 36 feet while accessing a roof to conduct re-roofing activities on a Snorkel TB-60 aerial lift. The employees did not attach a lanyard to the boom or basket of the aerial lift. 29 CFR 1926.453(b)(2)(vi): Boom and basket load limits specified by the manufacturer were exceeded:  (a) On or about June 11, 2018, and at times prior, White Roofing, Inc., as the controlling, creating, and exposing employer did not ensure that the basket load limits specified by the aerial lift manufacturer were not exceeded. Four employees of G&G Roofing, LLC., a subcontractor, were exposed to falls of up to 36 feet while accessing a roof to conduct re-roofing activities on a Snorkel TB-60 aerial lift. Three employees and the aerial lift operator weighting a total of 606 Lbs. exceeded the manufacturer load limits of the basket which is rated for two occupants and a maximum combined weight of 500 Lbs. 29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  (a) On or about June 11, 2018, and at times prior, White Roofing, Inc. did not ensure that each employee who operated an aerial lift was trained by a qualified person to recognize the hazards associated with aerial lifts and to understand the procedures to control or minimize those hazards. The operator of aerial lift Snorkel TB-60 had not been trained on this type of equipment, the operator hoisted other workers without a restraining system in the basket, the operator exceeded the capacity of the basket exposing employees of G&G Roofing, LLC., a subcontractor, to fall hazards of 36 feet.
