29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 964 West Richland Circle, Auburn, Alabama:  On or about June 12, 2018 and at times prior; the employer allowed his employees to use a ladder to gain access to the second floor and the rails of the ladder did not extend three feet above the landing exposing his employees to fall hazard of 10.4 feet to the concrete floor below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At 964 West Richland Circle, Auburn, Alabama:  On June 12, 2018; the employer continues to allow his employees to perform framing activities from the trusses without fall protection exposing his employees to fall hazard of 25 feet to the concrete floor below.  b) At 964 West Richland Circle, Auburn, Alabama:  On June 12, 2018; the employer continues allow his employees to perform roof decking activities from the trusses without fall protection exposing his employees to fall hazard of 18.6 feet feet to the ground below.
