29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours: a) Live Dock, Live Hang area: On or about September 14, 2018, the employer failed to notify the Occupational Safety and Health Administration (OSHA) within the specified twenty-four hour period of an inpatient hospitalization of a live hang employee that was hospitalized, tested positive for bacterium Chlamydia psittaci, and was diagnosed with Psittacosis on September 14, 2018. OSHA was notified by phone on September 18, 2018.
