29 CFR 1910.95(g)(6):At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) D Bone (Shaker): On or about November 7, 2024, an employee was exposed to noise hazards in that a machine operator was exposed to noise at an 8-hour time weighted average of 92.2 dba. This level exceeded the action level of 85 dba. The exposure was derived from a sample collected from a 370-minute sampling period. The exposure calculation included a zero increment for the 110 minutes not sampled. The employer has not provided the employee with annual audiometric testing. b) Freezer-Dicer Operator: On or about November 7, 2024, an employee was exposed to noise hazards in that a machine operator was exposed to noise at an 8-hour time weighted average of 87.6 dba. This level exceeded the action level of 85 dba. The exposure was derived from a sample collected from a 370-minute sampling period. The exposure calculation included a zero increment for the 110 minutes not sampled. The employer has not provided the employee with annual audiometric testing. c) D Bone Pack: On or about November 7, 2024, an employee was exposed to noise hazards in that a machine operator was exposed to noise at an 8-hour time weighted average of 88.3 dba. This level exceeded the action level of 85 dba. The exposure was derived from a sample collected from a 370-minute sampling period. The exposure calculation included a zero increment for the 110 minutes not sampled. The employer has not provided the employee with annual audiometric testing.
