29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: WORKSHOP/PROCESSING BUILDING: The employer had not established and implemented step-by-step hazardous energy control lockout/tagout procedures in the site and machine specifics, including alternative measures for the employees during servicing and maintenance of equipment, such as (but not limited to) the transfer conveyor. On/or about May 15, 2018, the transfer conveyor of the Bollagraf and Harris baler was jammed with residue/solid wastes, and as soon as an employee, while standing on the conveyor, attempted to unjam the conveyor using a foot, the conveyor moved forward causing foot injuries.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: WORKSHOP/PROCESSING BUILDING: The employees, as "authorized, affected, and other" types were not provided with re-training and information in the controlling of potentially hazardous and unexpected releases of energy sources (lockout/taggout) during servicing and maintenance of equipment, such as (but not limited to) the transfer conveyor.
