McCreary Roofing Company, 501 East 38th Street, Erie, PA 16546, roof area, east side of Weber Hall, on or about June 26, 2018 and times prior thereto:  Employees were using flexible electrical extension cords to power a DeWalt DC9310 Fast Charger to charge DeWalt DC9096 18V Battery Packs.  The flexible cord was not used in conjunction with a ground-fault circuit interrupter.  The company did not have an assured equipment grounding conductor program.
